This is a plant my friend bought at the Philadelphia plant show many years ago. It needs very little water. It grown very slowly.

This looks like a caudex, you can check out the genera of caudices to narrow its ID. I’m not an expert on these plants. I can only speculate, this might be a species in the Fockea genus? Best wishes! http://succulent-plant.com/families/apocynaceae/asclepiadaceae/fockea.html
